Please print all forms and complete all questions.  You may scan and email forms back to office@jbs.school.nz with all required** documentation or you can drop them in to the school office.

• ALL pre-enrolments are subject to approval by the School and you may be required to attend an interview with the Principal
• You MUST be living in our Enrolment Zone and proof of address must be provided**
• A Birth Certificate &/or Passport is required for all new enrolments **
• Immunisation Certificates are required for all children starting school as a new entrant (5yrs old)**
• You may be asked to attend a meeting at the school if we require further information
• Uniform & Stationery will be available to purchase once your enrolment has been confirmed
